Seasoned Recruitment is currently seeking a highly motivated Clinical Pharmacist I to join our team. In this role, you will be responsible for providing clinical pharmacy services to patients and healthcare providers. You will work collaboratively with a multidisciplinary team to optimize medication therapy and ensure the safe and effective use of medications.

Responsibilities:

• Review medication orders and evaluate their appropriateness based on patient-specific factors.
• Participate in the development and implementation of medication therapy management programs.
• Collaborate with healthcare providers to develop treatment plans and monitor patient responses to therapy.
• Provide drug information and education to patients and healthcare providers.
• Conduct medication reconciliation to ensure accurate medication records.
• Contribute to quality improvement initiatives and medication safety programs.
• Stay updated on new developments in clinical pharmacy practice and contribute to the implementation of innovative practices.
• Adhere to all relevant legal, regulatory, and safety requirements.

Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited institution.

• Pharmacy licensure in the state of practice.
• Minimum of 1-2 years of experience in clinical pharmacy, preferably in a hospital or healthcare setting.
• Strong clinical skills and knowledge of evidence-based medicine.
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary team.
• Experience in medication therapy management and medication reconciliation.
• Familiarity with relevant regulatory requirements and industry guidelines.
• Attention to detail and strong problem-solving abilities.
